IMPORTANT: For our activities we require that a Children's Health Form be completed. We keep these forms on file for one year, and then ask that you resubmit an updated Health Form the following year. You can download the form, print it off and either fax in your completed form (410-2337), mail it in (address at top of page) or drop it off in person. PAYMENT: Payment deadlines are posted on the activity descriptions. Please pay at the MFRC Office, where we accept cash, cheque and debit card. You may also mail us a cheque, provided that it arrives before the payment/registration deadline. Online payments are currently not availalbe, but will be reinstated at a later date. A waiver must be completed for every activity. The waiver can be downloaded here. Please submit completed forms as per instructions above.
